Electronic Application Submission For A Plumbing Permit

The Applicant understands and agrees that the applicant's signature on this application shall  constitutes  an agreement to comply with all District Ordinances:

1.    Certificate of Insurance (COI) validation is required before the commencement of work. This includes COI's being on file at the District Office  (attached to this application )and that effective dates are current and the appropriate endorsement is attached. Carmel Area Wastewater District must also be listed as additional insured. The COI must include the following:

  • Commercial Liability
  • Automobile Liability
  •  Worker's Compensation
  • There is an exception to the Workers Compensation requirement If the plumber is a sole proprietor and there are no employees the Worker's Compensation insurance is not required.

 2. Endorsement CG20-10 or CG20-13 (or State equivalent endorsement) must be attached to the COI.

3.    Permit expires 60 days from the date of issuance or earlier depending on the effective dates listed on the certificate of insurance on file.

4.    The applicant's signature on this application shall constitute an agreement to comply with all District Ordinances.

5. Permit expires 60 days from the date of issuance or earlier depending on the effective dates listed on the Certificate of Insurance (COI).
